Still Tired on Levothyroxine? 6 Reasons Your Thyroid Med Isn't Working
The diagnosis was almost a relief. Finally, a name for the exhaustion: hypothyroidism. You started the little white pill, your doctor retested in eight weeks, and the verdict came back — "your levels are perfect now."
Except you don't feel perfect. You feel exactly the same. Still flattened by 2pm. Still cold. Still watching the scale creep up on the same food that never used to touch you.
If the pill fixed the number but didn't fix you, this article is for you. Here are the six most common reasons levothyroxine isn't enough — and what you can actually do about each one.
First, the important bit: nothing in this article is a reason to stop, skip or change your medication. Levothyroxine is doing a real job. The point is that for many women it's necessary but not sufficient — and the gap between "treated" and "well" has specific, fixable causes. Keep taking your meds as prescribed, and bring this list to your doctor.
You are not an outlier, and you are not imagining it. When researchers surveyed more than 12,000 hypothyroid patients in one of the largest treatment-satisfaction studies ever run (published in the journal Thyroid in 2018), satisfaction with standard levothyroxine treatment was strikingly low, and people on T4-only therapy reported persistent symptoms — fatigue, weight gain, brain fog — despite "normal" lab results. Roughly one in ten to one in seven patients on levothyroxine continues to feel unwell even when their TSH sits obediently in range.
That's millions of women being told the problem is solved while they're still living the problem. The reasons below explain how both things can be true at once.
Reason 1: Levothyroxine is T4 — and T4 doesn't actually do anything
This is the piece almost nobody explains at diagnosis. Levothyroxine is synthetic T4 — the storage form of thyroid hormone. T4 is mostly inert. Before your body can use it, it has to be converted into T3, the active hormone that runs your metabolism, your body temperature, your energy, your mood and your weight regulation.
So the pill doesn't hand your cells energy. It hands them raw material and trusts your body to do the manufacturing. Your TSH normalises because the pituitary sees plenty of T4 circulating — which is exactly why TSH can look "perfect" while you feel awful — but if the conversion step downstream is broken, your cells never receive the finished product.
Which brings us to the question that actually matters:
Reason 2: Your body can't make the T4-to-T3 conversion
Conversion happens mostly in the liver, gut and kidneys, carried out by enzymes called deiodinases. Those enzymes are exquisitely sensitive to the overall state of your body — and they get suppressed by precisely the things exhausted women tend to be doing:
- Dieting. Severe calorie restriction measurably lowers T3 — it's one of the most replicated findings in metabolism research. Your body reads "famine" and deliberately slows the conversion to save energy. This is why crash dieting on a slow thyroid backfires so reliably.
- Low-carb and keto. The conversion step is glucose-dependent. Carbohydrate restriction reliably lowers active T3 — fine for a metabolically robust man, a real problem for a hypothyroid woman.
- Chronic stress. Cortisol redirects T4 down a different pathway (more on that next).
- Inflammation and gut issues. A significant share of conversion happens in the gut; inflammation suppresses the enzymes that do it.
The result: medicated, "normal" labs, and a metabolism still running cold. If you want a quick reality check on this, your body keeps its own scoreboard — waking temperature and resting pulse. A waking temperature stuck below 36.5°C is a strong sign the active hormone isn't getting through, whatever the bloodwork says.
Reason 3: You're taking it in a way that blocks absorption
This one is mundane, fixable this week, and shockingly common. Levothyroxine is a fussy molecule — its absorption is easily wrecked by what's in your stomach alongside it:
The absorption killers
Coffee (yes, even with milk — especially with milk), calcium in any form (dairy, supplements, fortified plant milks), iron supplements, magnesium, antacids, and high-fibre breakfasts taken within an hour of your tablet can cut absorption substantially. If you swallow your pill with your morning latte, a real fraction of your dose never makes it into your bloodstream.
How to take it properly
Empty stomach, plain water, then wait 30–60 minutes before coffee or breakfast. Keep iron, calcium and magnesium supplements at least 4 hours away from your dose. Same time every day. Reason 4: Reverse T3 is parking in your T3's spaces
When your body is under chronic strain — stress, illness, under-eating, over-exercising — it doesn't just slow T3 production. It actively converts T4 into reverse T3 (rT3): a mirror-image molecule that docks in T3's cellular receptors but does nothing. It's biological traffic management — your body deliberately parking dead hormone in the active spaces to force a slowdown.
Here's the cruel twist for medicated women: more levothyroxine means more T4 available — and if your body is stressed, that extra T4 gets shunted into more reverse T3. The dose goes up, the TSH looks even better, and you feel worse. If that pattern sounds familiar, ask for reverse T3 to be included in your next panel; it's the test that catches this, and it's almost never run by default.
Reason 5: You're missing the nutrients the conversion runs on
The deiodinase enzymes that turn T4 into T3 are built around specific nutrients. Run low on them and the machinery slows regardless of how much T4 you swallow:
- Selenium — the core atom of the conversion enzymes. Food sources: 2–3 Brazil nuts a day, fish, shellfish, eggs.
- Zinc — needed for both conversion and receptor sensitivity. Food sources: oysters (the champion), beef, lamb, cheese.
- Iron (ferritin) — low ferritin is rampant in menstruating women and quietly sabotages thyroid function; many practitioners want ferritin comfortably above 50 before expecting thyroid treatment to feel like it's working. Food sources: red meat, liver.
- Vitamin D and B12 — both commonly low in hypothyroid and Hashimoto's patients, both cheap to test, both worth knowing.
Note what this list is: food. Not exotic supplements — the same nourishing, pro-metabolic foods that support thyroid function generally. A woman on a beige low-calorie diet of cereal, salad and protein bars can be swallowing a perfect dose of levothyroxine into a body that lacks every cofactor needed to use it.
Reason 6: It's Hashimoto's — and the fire is still burning
The majority of hypothyroidism in women is Hashimoto's thyroiditis — an autoimmune condition in which your immune system attacks your thyroid. Levothyroxine replaces the hormone your damaged thyroid can't make. It does nothing about the immune attack itself.
Many women are medicated without ever being told they have Hashimoto's — antibodies often aren't tested once the TSH is high enough to medicate. But it matters enormously, because the autoimmune process produces its own symptoms (flares of fatigue, aches, brain fog, anxiety) that no amount of T4 will touch, and because the inflammatory side of Hashimoto's responds to food — removing inflammatory polyunsaturated seed oils, supporting the gut, eating enough to lower the stress load.
If you've never had anti-TPO and anti-thyroglobulin antibodies tested, that's the first thing to ask for. It reframes the whole project: not "replace the hormone and done," but "replace the hormone and calm the system attacking the gland."
What to ask your doctor for: a full panel — TSH, free T4, free T3, reverse T3, anti-TPO and anti-TG antibodies — plus ferritin, vitamin D and B12. And if your free T3 sits low while everything else looks "fine," ask directly: "Would a trial of combination therapy (adding T3/liothyronine) make sense for me?" Some women do dramatically better on it. It's a prescriber's call — but it's a conversation you're entitled to open, with data on the table.
What you can do from the kitchen (alongside the prescription)
Look back at the six reasons and notice something: apart from the absorption mechanics and the dosing conversation, almost every one runs through the same bottleneck — a body too stressed and under-fuelled to convert and use the hormone it's being given. That's the part you own. Not by restricting harder, but by the opposite:
- Eat enough — especially carbohydrate. Fruit, root vegetables, white rice, honey, milk. Glucose is what funds the T4→T3 conversion. Chronic undereating is the single most common self-inflicted conversion block.
- Stop fasting. Long fasting windows raise cortisol and reverse T3 — the exact pattern from Reason 4.
- Strip out polyunsaturated seed oils (sunflower, canola, soybean, "vegetable" oil) — they suppress thyroid hormone activity at the cellular level. Cook in butter or coconut oil instead.
- Cover the cofactors with food: a few Brazil nuts, regular red meat or liver, shellfish when you can, eggs, plenty of dairy.
- Track your waking temperature. It's the cheapest feedback loop in medicine: if your morning temperature climbs toward 36.5–37°C over weeks of eating this way, the conversion is recovering — whatever the next TSH says.
